[City of Mountain House] REC-009 Nonprofit Organization Verification Form

Thank you for submitting your organization's Non-Profit Verfication Form. Please attach a copy of the organization's current IRS tax exempt letter and current roster.

The following items must be submitted with the “Non-Profit Verification Form.”

The organization must be “Active” with the California Secretary of State (SOS).
Current IRS tax Exempt Letter, from an organized non-profit group with current 501(c)(3) or 501(c)(4) IRS status.
Member list with at least 75% of the organization’s members reside within the Mountain House’s boundaries (95391).
Organizations whose roster is 75% or more City residents and whose principal address is in City (95391) will receive priority over organizations with principal address outside of City (95391)

Once your non-profit has been verified, the organization will be flagged as a non-profit and may receive the benefits of being classified as Group B: Sanctioned non-profit with the City of Mountain House.
